Analysis of the Implant Induced Bending Stresses Across the Maxillary Sinus Floor

Abstract
Expansion of the maxillary sinus entails a decreased bone height at its floor, thus prohibiting placement of dental implants of reasonable sizes and producing higher bending moment. Investigation of the stress around dental implants placed at the floor of the maxillary sinus and at the boundary of the expanded maxillary sinus. ITI solid implants of 6mm in length and 4.8mm in diameter placed at the floor of pneumatized maxillary sinus were axisymmetrically modeled for a series of finite element analysis. Main consideration was given to the stresses induced directly by occlusal load as well as the bending moment incurred stresses as a result of the increased size of maxillary sinus. Depending on the lateral diameter of the maxillary sinus, different models were set for analysis. Vertical displacement of the implant, stresses at the cervical and the apical cortical bone near the implant and at the boundary of the maxillary sinus were evaluated for comparison. Highest stress was observed at the cervical area of all the models. The peak stress at the cervical bone and at the maxillary sinus boundary increased as a function of the size of the maxillary sinus. Stress at the apical bone decreased as the size of the maxillary sinus increased. The level of stress at the three areas of stress concentration was not significant. The increased size of the maxillary sinus up to 32mm in diameter did not pose a threat to the success of the implant.
